The Taneda Karate Dojo will start issuing receipts for the Children's Fitness Tax Credit starting january 1st 2010. These receipts need to be very detailed and must be done correctly. Please help us by making sure all our data is correct in our computer by filing out this quick request form.

If you are submitting this request for more than one child please list all their names in the first box and list their birthdates in the same order in the Birth date box. Please enter birth dates in this format: Jan/22/1999

Please allow 1 week to process your request for a tax receipt. All receipts will be emailed to the address suplied in the request form. If you would like a hard copy please let our office know.

Eligible fees:
- Training dues - Fees paid within the calendar year (Jan 1- Dec 31). Even if they aren't used in that calendar year. Example: Fees pre-paid for Jan-Mar 2010 are eligible for the 2009 tax credit if they were pre-paid before December 31 2009. All fees paid now for Jan-Mar 2010 will be eligible for next year.
