Tax Receipt Request Form

The Taneda Karate Dojo will start issuing receipts for the Children’s Fitness Tax Credit starting January 1st 2012. These receipts need to be very detailed and must be done correctly. Please help us by making sure all our data is correct in our computer by filing out this quick request form.

If you are submitting this request for more than one child please list all their names in the first box and list their birthdates in the same order in the Birth date box. Please enter birth dates in this format: Jan/22/1999

All receipts will be emailed to the address supplied in the request form. If you would like a hard copy please let our office know.
